  2. What brought you to seek help at this time?
  3. What are some of the challenges you’re currently facing?
  4. How have these challenges impacted your life?
  5. What are your goals and aspirations for the future?
  6. What are some of your strengths and coping mechanisms?
  7. Are there any cultural or religious practices that are important to you?
  8. How does your family or support system fit into your life?
  9. Have you ever experienced trauma or abuse? If so, would you like to talk about it?
  10. What do you think might be the best way for us to work together to achieve your goals?
